Every three years, Dartmouth Hitchcock and Alice Peck Day Hospital conduct a Community Health Needs Assessment with other local providers in the region. We use this assessment to better understand what needs our community is most concerned about and what conditions are impacting the health and well-being of our region's residents. This assessment helps inform hospital services and our community health initiatives, and is widely used by community organizations in planning services.
We want to hear from YOU! Please take 10 minutes to give us your thoughts and opinions. Please use the link below to complete the survey.
Data from the needs community needs assessment, key stakeholder surveys and input from local discussion groups can be found on the Dartmouth Hitchcock website in the fourth quarter of 2018.
Thank you for your time,
